How do I schedule an appointment?

What is your cancellation policy?

If you miss an appointment or fail to cancel prior to 48 hours prior to your appointment, you will be charged a $100 cancellation fee.

If you miss an appointment, you will be charged the full cost of the scheduled visit.

Do you take my insurance?

No. Dr. Zaklin does not accept any insurance plans for his practice, nor are his services billable as an out of network provider. All patients must sign a waiver agreeing not to submit a claim to their insurance carrier for their visits.

What forms of payment are accepted at your office?

Cash, Credit ( MasterCard, Visa, American Express, Discover), or Paypal. Dr. Zaklin will also accept payment in the form of BTC and ETH based on current market value. No personal checks please.

Do you review Medical records?

Yes. Medical records can only be released with your authorization. It is your responsibility to obtain previous medical records from other physicians or health care providers that you wish to be reviewed. For further details, go here.

Does being evaluated guarantee certification for the Medical Use of Marijuana?

No. Being evaluated for The Medical Use of Marijuana program does not guarantee certification. Be sure to review the regulations including qualifying conditions prior to your appointment here. Specific questions can be addressed here.

Are you a primary care practice?

No. Dr. Zaklin does not serve as a primary care physician within the scope of this practice. As a result, he does not address urgent care or primary care issues. We recommend that you have a primary care physician (PCP). If you have a serious health problem that requires immediate attention, you should call your PCP, call your specialist, call 911, or have someone take you to the nearest hospital emergency room. If you notice an adverse effect from one of the components of your health plan, you should discontinue use immediately and then contact the office during regular business hours.

Is there anything I should do prior to my first visit?
